Tenant Application
The application fee is $15.00 and is non-refundable. I hereby authorize the landlord to verify any and all information on this application, criminal report and/or credit report. I/We, the undersigned, authorize United Properties of West Michigan to obtain credit information from Merchant Service Bureau/CBC Companies, other Credit Bureau, or any other screening service, which MAY include credit history, rental history, criminal history, sexual offender history and terrorist information for the purpose of dertermining eligibility to lease an apartment or house or to obtain collections information.
